Liability Waiver

In consideration of being allowed to participate in the sports programs and activities provided by Sports X, I, the undersigned participant (or parent/guardian of the participant if the participant is under 18 years old), hereby acknowledge and agree to the following

I understand and acknowledge that there are inherent risks associated with participating in sports activities, including but not limited to, the risk of physical injury, illness, and property loss. I voluntarily assume all such risks and agree to release, indemnify, and hold harmless Sports X, its owners, employees, agents, volunteers, and affiliates from any and all claims, demands, damages, or liabilities arising out of or related to my participation in the sports programs and activities, regardless of whether such claims are caused by negligence or otherwise.

I certify that I (or my child if I am the parent/guardian) am physically fit and capable of participating in the sports programs and activities offered by Sports X. I acknowledge that it is my responsibility to consult with a physician or other healthcare provider before participating in any physical activities if I have any concerns about my (or my child's) physical condition.

I agree to comply with all rules, regulations, and instructions provided by Sports X and its staff during my participation in the sports programs and activities. I understand that failure to comply with such rules, regulations, and instructions may result in my removal from the programs and activities without refund.

I authorize Sports X and its authorized representatives to obtain emergency medical treatment for me (or my child) in the event of any injury, illness, or medical emergency that may occur during my participation in the sports programs and activities. I understand that Sports X will make reasonable efforts to contact me (or my emergency contact) in the event of a medical emergency, but I agree that Sports X shall not be liable for any delay in obtaining medical treatment or for any injury, illness, or other damages that may occur as a result of such delay.

I have read and understand this liability waiver and agree to be bound by its terms and conditions.
